Etan's Journey provides crucial adult medical and mental health respite care for individuals who are medically stable but lack a safe place to recover following illness, injury, and hospitalization. Located in Charlotte, North Carolina, our proposed 10-bed residential program is designed to support vulnerable adults. We aim to reduce hospital readmissions, improve post-acute recovery outcomes, and promote long-term housing and health stability among those experiencing homelessness or housing instability. Who can refer patients to Etan's Journey?
Referrals to Etan's Journey are accepted from a wide range of organizations and individuals, including: Hospitals and health systems (discharge planners, case managers, social workers), Emergency departments, Managed Care Organizations (MCOs) and Medicaid care managers, Community Clinics and Federally Qualified Health Centers (FQHCs), Behavioral health providers, Homeless service providers and outreach teams, and Individuals or family members seeking guidance or support.

Our 24/7 clinical and supportive care, led by Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Licensed Clinical Social Workers (LCSWs), Licensed Clinical Mental Health Counselors (LCMHCs), and Case Managers, ensures comprehensive support. We focus on improving post-acute recovery, reducing hospital admissions, and promoting long-term health and housing stability.
How do i refer a patient or admit a loved one?
Our preferred referral method, once active, will be through NCCare 360, North Carolina's coordinated care network. Alternatively, you may initiate a direct referral by contacting Etan's Journey directly at (704)208-6429. Our team will follow up promptly to review eligibility and guide you through the next steps.

What information is needed for a referral?
To support a safe and timely admission, we request the following information: Basic demographic information, Hospital discharge summary or referral documentation, Current medication list, Relevant medical or mental health care instructions, and Insurance or Medicaid status (if available). Etan's Journey will collaborate with referring providers to obtain any additional necessary information.
What kind of patients will benefit most from Etan's Journey?
We serve adults who are 18 years or older, medically stable and do not require inpatient or skilled nursing care, are experiencing homelessness or housing instability, can participate in basic self-care with support, and have a plan for follow-up medical care.
